Dementia Peer Support Coordinator

Job Description – Dementia Peer Support (Peer Support Service)

Title/Role:  Dementia Peer Support Coordinator

Peer support service:

This CCG funded project is an 18 month pilot programme offering opportunities and the development of skills for people living with dementia and their family carers within the borough of Brent. The project recognises that people with a diagnosis of dementia and their family carers have talent, work/life skills and experiences. We want to harness these skills to support and empower their diagnosed peers with dementia and share learning on living well with dementia through the provision of advice, support and the development of activities that are culturally relevant, engaging, enjoyable and empowering.

Our current coordinator is moving on to a senior NHS post therefore we are seeking to recruit someone who can continue the excellent work already undertaken. We are looking for a coordinator with an empowering, inclusive approach to recruit, support and harness the skills of people with dementia who are experts in living with the condition. Peer Supporters can in turn empower and support their peers to live well and independently with dementia in the community within a social model of care.
