Health & Wellbeing checks and advice project Lead

10 hours a week during office hours. Times to be negotiated with successful candidate.

  • six month contract.
  • £14  per hour
  • place of work. 60 Ashford Road, NW2 6TU
  • 25 days plus 8 days public holidays pro-rata

We are looking for an ambitious and experienced Health professional who is passionate about improving the health and wellbeing of all in our local community. Do you want to lead on the development of a health and wellbeing community programme? This pilot project aims to identify the health care needs of residents for whom health services have traditionally found it difficult to connect with. With the results of health checks along with demographic data we plan to design community based health and welfare services for Brent residents.

With this in mind Ashford Place is undertaking a 6 month pilot project to

  1. Encourage residents to take up health checks provided by you, especially those who are homeless, living with mental health challenges, BAME communities and other communities experiencing health inequalities
  2. Signpost clients to services/agencies that provide support and advice services within Ashford Place and externally. 
  3. Enhance communication links between services to ensure best possible outcomes for beneficiaries.
  4. Collate and record health check results and demographic data throughout the project timeline.

Qualifications                                                                      

Professional qualification. HCA or healthcare professional

Skills/Knowledge/Abilities/Experience

  1. Experience of working with residents that are traditionally under served by health services                
  2. To have an awareness of Equal Opportunities                           
  3. To be able to demonstrate an understanding risk management,
  4. in relation to both practice and the environment of care
  5. To have the ability to coordinate health promotion initiatives
  6. To be able to work both independently and as part of a wider team
  7. To be an effective communicator verbally and in writing
  8. To be able to perform basic word processing tasks
