Keep Well: Supporting Health and Wellbeing in Brent
We are in a new phase of our keep well project.
Our Keep Well Project aims to reduce hospital stays by supporting community discharges, and enhancing the independence, wellbeing, and health of our local community. We focus particularly on those facing low-severity mental health concerns, learning disabilities, and dementia.
Mission: We empower individuals to manage their health needs and access essential services through a preventative community care model. By collaborating with hospital discharge teams, GPs, healthcare professionals and community organisations, we promote available services, facilitate self-referrals, and serve as a community hub to prevent avoidable ill health.
Services: This project encapsulates our work in preventative care. We promote social inclusion, support health and wellbeing, and engage with the local community.
Our efforts help to keep people well, to feel socially connected, and provides a friendly place to meet, eat, and chat. To learn more about our services, individuals can self-refer or be referred to us by an organisation acting on their behalf.
Our Services: Keep Well offers a range of services to support the community, including:
- Facilitating connection to social support groups, peer groups, and workshops
- Assistance with annual health checks (GPs, opticians, dentists, etc.)
- Information on services and activity groups to boost physical health
- We assist in providing guidance for a variety of wellbeing concerns and offer tailored recommendations.
- Tips for healthy eating, exercise and reducing loneliness
Dementia Peer Support Project: Enabling people living with dementia to lead on positive solutions.
Dementia Cafes: Supporting a dementia-friendly borough.
Social Activities Programmes: Including counselling, fitness, and nutritious meals.
Community Café: A welcoming space for nutritious lunches and various activities.
Counselling Support: 120 already people provided with counselling support.
Fit and Well Programme: 145 active members.
Local Heroes: Supporting 10 local heroes who deliver health & wellbeing support.
Nutritious Meals: 4000 healthy lunches served in our community café.
Side Door Café: Offering a cuppa and social connection.
Why This Matters: Brent is among the top 15% of the most deprived areas in the country, with high levels of poverty affecting health and wellbeing. Our programmes aim to increase awareness of best practices for quality nutrition, positive mental health, and healthy lifestyles.
Mental Health: 1 in 5 adults have a common mental disorder. We aim to help more people find the support they need.
Dementia: Affects 1 in 14 people over 65 in the UK. In Brent, it is estimated over 2000 people are living with dementia currently.
Learning Disabilities: Approximately 7000 people in Brent have a learning disability, increasing the risk of mental illness.
Older Adults: Often face loneliness, dementia, and physical challenges. Our support is crucial as Brent’s population ages.
