Project Keep Well is a programme of support to vulnerable residents in Brent that both supports them to make positive lifestyle changes, to effectively access statutory and community services that they have a right to, and also to help them avoid the need to access A&E and hospital services. We all recognise that the NHS is under pressure and equally, that there are support systems in the community that should be, ideally the first choice for vulnerable people. We will support people to make this happen, help them to effectively connect with health and social welfare related services and activities, and finally help them to find ways of being better able to manage their vulnerabilities. The main groups of people we can support are those living with mental health challenges, living with dementia and people with learning disabilities. We are pleased to be working with the NorthWest London Clinical Commissioning Group.
